What's Included:
Students are presented with a series of 6 statements per component of health.  They are to evaluate if that statement is true for them or not.  They will add up their total number of yes statements. 
 
Students are then asked which area of health they scored lowest in.
They are then asked to identify ways to improve upon that component of health.
